Hello,
I’m developing/revamping a site for my client and having a similar problem as other but downgrading to 1.4.9 does not solve the issue. The form doesn’t display on products with variations. It displays on simple products only.
I’m using:
Plugin version 1.5.1
Theme – Shopkeeper: Version 2.6.11 https://shopkeeper.wp-theme.help/hc/en-us/articles/207365265-Updates-History-Changelog-
WordPress: version 4.9.8
Also using a Variation swatches plugin latest version: 1.0.3 https://wordpress.org/plugins/variation-swatches-for-woocommerce/All the above (minus the theme) worked on my clients old site.

Can you please share the theme file with us overdrive, so we can test the same on our end.
Also, please do share a temporary admin and FTP access also, so we can inspect the issue too.
While sharing the access, don’t forget to mark the response as private.
